By submitting this form, I certify that I have read the roof garden rules. I understand that any violation of the roof garden rules will result in a $2,500 fee for use of the the roof garden.

I also understand that an employee of the building must be present during the party, and that the labor cost (4 hours @ $50/hour minimum) will be billed to me.

The board will send its approval (or disapproval) of the party within two weeks.
